Total Care Therapy is seeking a compassionate Physical Therapist to join our growing team in Avon, Indiana. You will evaluate, develop, and implement individualized plans to improve mobility, reduce pain, and maintain independence for residents in assisted living communities.

Qualifications

• Current State Physical Therapist license.
• CPR/BLS certification.
• Valid driver's license and reliable transportation.
• Experience in geriatrics, home health, assisted living or skilled nursing preferred.
• Leadership experience preferred.

Responsibilities

• Provide skilled PT services per established plans of care.
• Deliver therapeutic exercise, gait, balance and mobility interventions.
• Monitor progress and adjust treatment plans to meet goals.
• Supervise PTAs per state regulations.
• Complete timely EMR documentation.
• Educate residents and caregivers on home programs and safety.
• Collaborate with interdisciplinary team for coordinated care.
• Maintain a productive clinical caseload with compassionate care.
