Position Summary

Population health management involves understanding social, economic, familial, cultural, and environmental factors affecting health outcomes across different locations and groups (socioeconomic, racial/ethnic, age). It includes managing physical, dental, mental health, and social needs.

This is a fully remote position, eligible for candidates residing in Michigan.

Core Responsibilities
  • Manage all components of population health management.
  • Design, implement, and evaluate population health initiatives based on scientific principles.
  • Champion system-wide initiatives supporting equitable, whole-person care for enrollees.
  • Develop operational plans addressing market opportunities and challenges aligned with population health goals.
  • Coordinate with Medical Director, Quality Improvement, Utilization Director, and external parties on population health activities.
Required Qualifications
  • Reside in Michigan.
  • 7-10 years of clinical and/or managed care experience in healthcare.
Preferred Qualifications
  • Active, unrestricted Michigan registered nursing license.
  • Proven track record or proficiency in relevant competencies.
  • Strong communication and computer skills.
  • Ability to work in a fast-paced environment.
  • Ability to plan, organize, and execute multiple objectives.
  • Excellent communication skills for interacting with diverse audiences, including senior management.
  • CPHQ or other relevant certification is a plus.
Education

Master’s degree or higher in nursing, social work, health services research, health policy, IT, or related fields.

Pay Range

The typical pay range is $100,000 - $231,540, depending on experience, education, and location. This role may include bonuses, incentives, and equity awards.
