Position Summary

Leads field staff in delivering exceptional client service, clinical care, quality, and adherence to policies and procedures. Responsible for internal case management, staff evaluation, and development. Conducts on-site assessments to identify client needs, plans services, monitors care plan implementation, evaluates staff performance, participates in training, and assumes clinical management to ensure services align with the organization's mission.

Primary Responsibilities

1. Demonstrate and communicate the core values of CareTen, Inc.
2. Show high flexibility in collaborating with the service office team.
3. Maintain high client satisfaction scores.
4. Exhibit expertise in conflict management skills.
5. Conduct client assessments and manage care planning, including development, implementation, and updates.
6. Handle clinical and client case management.
7. Provide clinical oversight to field staff, including Certified Home Health Aides and LPNs.
8. Support the team by providing clinical guidance, representing CareTen in community activities, overseeing quality improvement, participating in meetings and events, and performing related duties as requested.
9. Assist in agency evaluation activities.
10. Participate in relevant state and national organizations, meetings, seminars, and workshops.
11. Review policies related to patient care with the Professional Advisory Committee.
12. Oversee CQI and utilization review activities.
13. Implement and evaluate client care services, ensuring compliance with licensure, certification, and accreditation standards.
14. Be available during operating hours with flexibility for after-hours and weekends.
15. Manage service and employment-related activities outside normal hours as needed.
