Job summary

Middle River Health and Rehabilitation Center, an 86-bed skilled nursing and rehabilitation facility in Wisconsin, is seeking an experienced Director of Nursing to lead our nursing department and uphold our culture of quality care.

The ideal candidate will hold an active Wisconsin RN license, demonstrate nursing leadership in long-term care, and ensure survey readiness and compliance with CMS regulations, QAPI, and survey processes.

Qualifications

• Active RN license in Wisconsin.
• Nursing leadership experience in skilled nursing or long-term care.
• Working knowledge of state and CMS regulations, QAPI, and the survey process.

Responsibilities

• Lead nursing department to uphold high-quality care.
• Build and mentor a strong nursing team.
• Maintain survey readiness and regulatory compliance.
• Oversee daily operations and staffing in a 86-bed facility.
