Job summary

SNFout is seeking an experienced Director of Nursing for our 99-bed skilled nursing facility in San Gabriel Valley, CA. This full-time on-site leadership role offers a meaningful impact on resident care and compliance with CDPH and CMS regulations.

You will oversee RNs, LVNs and CNAs, drive staffing, quality metrics, and collaboration with the Administrator and Medical Director on resident outcomes. Competitive compensation and benefits available.

Qualifications

• Active California RN license.
• 3+ years of nursing leadership in a skilled nursing facility.
• Strong knowledge of Title 22, CMS regulations and survey process.
• Proven record of improving clinical outcomes and staff retention.
• Excellent communication and leadership skills.

Responsibilities

• Lead and develop the full nursing team (RNs, LVNs and CNAs) across all shifts.
• Own clinical quality, survey readiness and compliance with CDPH and CMS regulations.
• Oversee care plans, MDS accuracy and clinical documentation.
• Partner with the Administrator, Medical Director and IDT on resident outcomes.
• Drive staffing, scheduling, recruitment and retention of nursing staff.
• Lead QAPI, infection prevention and in-service education.
