Job summary

Apploi is seeking an experienced Director of Nursing for a 99-bed skilled nursing facility in the San Gabriel Valley, CA. This is a full-time, on-site leadership role with significant impact on resident care and team development.

You will oversee RNs, LVNs and CNAs, ensure CDPH and CMS compliance, and drive quality outcomes and staff retention. A California RN license and prior SNF leadership are required.

Qualifications

• Active California RN license.
• 3+ years of nursing leadership in a skilled nursing facility (DON, ADON or DSD).
• Strong knowledge of Title 22, CMS regulations and the survey process.
• Proven record of improving clinical outcomes and staff retention.
• Excellent communication and leadership skills.

Responsibilities

• Lead and develop the full nursing team (RNs, LVNs and CNAs) across all shifts.
• Own clinical quality, survey readiness and compliance with CDPH and CMS regulations.
• Oversee care plans, MDS accuracy and clinical documentation.
• Partner with the Administrator, Medical Director and IDT on resident outcomes.
• Drive staffing, scheduling, recruitment and retention of nursing staff.
• Lead QAPI, infection prevention and in-service education.
