Avir Trilogy in Texas seeks a Director of Nursing to lead the Nursing Services Department, planning and directing day-to-day operations while ensuring survey readiness and compliance with federal, state, and local rules. This role shapes the care culture by setting expectations for resident outcomes and staff accountability.

The Director will supervise the nursing team, coordinate quality initiatives, and participate in committees to improve care delivery and safety across the facility.

Qualifications

• Current Texas RN license
• Minimum one year supervisory experience in a healthcare setting
• Working knowledge of long-term care standards and federal/state regulations
• Ability to work independently and remain calm under pressure
• Patience, tact and positive attitude with residents and staff

Responsibilities

• Direct nursing operations to meet federal, state, and regulatory care standards and keep survey readiness.
• Lead the nursing management team and drive ownership of quality goals.
• Oversee continuous quality assessment and improvement of care delivery; keep administration informed.
• Develop and implement action plans supporting resident assessments and care plans.
• Perform daily rounds to verify care standards and monitor safety; report incidents.
• Audit charts for accuracy; identify issues and implement corrective actions.
• Maintain staffing levels through scheduling and shift approvals.
• Serve on Infection Control, Quality Assurance, and Utilization Review committees.
• Participate in Infection Control program, monitor hand hygiene, dressing changes, and medication passes monthly.
• Review resident complaints and document actions taken to resolve them.
• Monitor weekly skin assessments and wound reports; participate in on-call rotations

Avir Trilogy is seeking a Director of Nursing to join our team!

We're looking for a Director of Nursing who leads from the floor, not from behind a desk. You'll plan, organize, and direct the day-to-day work of our Nursing Services Department, keeping us survey-ready and in step with federal, state, and local regulations. Just as important, you'll build a nursing team that takes real ownership of resident outcomes, because the tone you set is the care our residents feel every day.

If you're a licensed RN with supervisory experience and you want a role where your judgment carries weight, we'd like to talk.

What You'll Do:

• Direct nursing operations to meet federal, state, and regulatory care standards, and keep the facility survey-ready year round.

• Lead the nursing management team and build ownership of quality goals across the department.

• Run continuous quality assessment and improvement of nursing care delivery, and keep the Administrator current on department activity and issues.

• Help develop and implement plans of action that support each resident's comprehensive assessment and care plan.

• Make daily rounds to confirm care standards are being met and to monitor safety, reporting unsafe conditions and all incidents immediately.

• Audit charts for accuracy and completeness, identify problem areas, and implement corrective action.

• Maintain appropriate staffing levels through work assignments, schedule adjustments, and shift approvals.

• Serve on facility committees including Infection Control, Quality Assurance, and Utilization Review.

• Participate in the Infection Control program, monitoring hand hygiene, isolation technique, dressing changes, linen handling, and medication pass at least monthly.

• Review resident complaints and grievances and document the actions taken to resolve them.

• Monitor weekly skin assessments and bath sheets, and complete the weekly wound report.

• Participate in the nursing on-call rotation.

Qualifications for Director of Nursing:

• Current, unencumbered Registered Nurse (RN) license in the state of Texas.

• Minimum of one year of supervisory experience in a hospital, skilled nursing facility, or related healthcare setting.

• Working knowledge of long-term care operational standards under the Federal Register, Conditions of Participation, and state regulations.

• Ability to work independently, exercise sound judgment, and remain calm under pressure.

• Patience, tact, and a positive attitude with residents, families, and staff.

• Comfort working with residents who are ill, disabled, elderly, or at times upset.

• An active role requiring standing, walking, bending, kneeling, and stooping throughout the day.
