Job summary

Health Dimensions Group seeks a Director of Nursing to lead clinical operations at Spring Valley Health and Rehab in Wisconsin. You will oversee nursing services, drive quality and compliance, mentor a nursing team, and partner with leadership to ensure outstanding resident care.

Qualifications

  • Graduate of an accredited school of nursing.
  • Current Registered Nurse license in Wisconsin, or ability to obtain Wisconsin licensure.
  • CPR certification required.
  • Strong knowledge of infection prevention and clinical best practices.

Responsibilities

  • Oversee and direct nursing and clinical services across the senior campus, ensuring residents receive high-quality, person-centered care.
  • Provide clinical leadership across Skilled Nursing, Transitional Care, Assisted Living, and Independent Living services.
  • Ensure resident assessments, care plans, treatments, physician orders, and clinical documentation are completed accurately and timely.
  • Monitor clinical outcomes, quality measures, and resident trends to identify opportunities for improvement.
  • Promote a culture centered on dignity, safety, independence, and quality of life.
  • Recruit, hire, train, mentor, and retain nurses, nursing assistants, and other clinical team members.
  • Create a cohesive, accountable, and high-performing nursing department.
  • Provide regular feedback, coaching, education, and professional development opportunities.
  • Establish clear expectations while supporting employees with the resources needed to succeed.
  • Foster a positive, collaborative culture that reflects the mission and values of Lutheran Living Senior Campus.
  • Maintain ongoing readiness for state and federal surveys.
  • Ensure nursing services comply with regulatory requirements, organizational policies, and clinical best practices.
  • Lead Quality Assurance and Performance Improvement initiatives to strengthen resident outcomes and operational performance.
  • Oversee audits, plans of correction, incident investigations, infection prevention, and clinical follow-up.
  • Partner with medical directors, physicians, pharmacy consultants, therapy providers, and regulatory agencies as needed.
  • Collaborate closely with the Executive Director, department leaders, and regional support teams.
  • Support staffing strategies, labor management, budgeting, and broader campus planning.
  • Strengthen interdisciplinary communication and ensure continuity of care across service lines.
  • Participate in care conferences, family meetings, leadership meetings, and community initiatives.
  • Serve as a trusted clinical resource and advocate throughout the campus.
  • Build meaningful relationships with residents and their families.
  • Ensure questions, concerns, and changes in condition are addressed promptly and compassionately.
  • Promote open communication and help families understand resident care plans and clinical needs.
  • Foster a welcoming environment that supports residents’ physical, emotional, and social well-being.

Lead Clinical Excellence

What You Bring to the Table (Besides Your Clinical Expertise)
Education and Licensure
  • Graduate of an accredited school of nursing.

  • Current Registered Nurse license in the State of Wisconsin, or ability to obtain Wisconsin licensure.

  • CPR certification required.

  • Strong knowledge of infection prevention and clinical best practices.

Leadership and Clinical Experience
  • Minimum of two to three years of nursing leadership experience, preferably within skilled nursing, transitional care, or long-term care.

  • Previous Director of Nursing, Assistant Director of Nursing, Unit Manager, or comparable clinical leadership experience preferred.

  • Demonstrated experience leading nursing teams, managing staffing, and developing employees.

  • Proven ability to oversee clinical systems, resident care, and regulatory compliance.

Clinical and Operational Knowledge
  • Strong knowledge of Wisconsin state and federal long-term care regulations.

  • Experience with survey preparation, plans of correction, clinical audits, and QAPI initiatives.

  • Strong assessment, care-planning, documentation, and clinical decision-making skills.

  • Ability to use quality data and clinical metrics to improve resident outcomes.

  • Excellent organization and prioritization skills in a fast-paced environment.
