Description

A Specialty Path to Good Health
Upperline Health is the nation’s largest provider dedicated to lower extremity, wound and vascular care. Founded in 2017 with the ambitious goal of changing specialty care, Upperline Health delivers a more efficient path for patients to receive consistent and effective treatment for chronic illnesses.

Triage is temporary.
Treatment is transformative.

When patients see their specialists at Upperline, they are provided access to a comprehensive care team that ensures patients receive the care they need. Our providers coordinate patients’ care among a team of specialists - physicians, advanced practice providers, care navigators, pharmacists, dieticians, and social workers for integrated treatment that addresses patients’ immediate and long-term health needs. Upperline Health is based out of Nashville, TN and has clinics in Alabama, California, Florida, Georgia, Indiana, Kentucky, and Tennessee.

We put patients at the center of value-based care.

Preferred candidates reside in/around Nashville but also open to considering applicants who reside in the following states: AL, GA, FL, KY, IL, IN, PA, SC, TN, and TX. Travel expectations are 30-40% to Nashville and various markets in the US.

About the Director of Implementation and Integrations Role
Upperline Health is seeking a highly motivated and detail-oriented Director of Implementation and Integrations to lead the integration of new clinics and partnerships into our rapidly growing network. Reporting directly to the Chief Operating Officer (COO), this role is central to Upperline Health’s expansion strategy and involves developing and executing seamless implementation processes across various markets. The Director will act as the primary liaison between internal teams, executive leadership, and external partners, ensuring all integration activities align with the company’s goals and timelines.

This fast-paced, operations-focused role requires exceptional project management skills, strong interpersonal and relationship-building abilities, and a results-driven mindset. The successful candidate will create implementation playbooks, advise leadership on deal structures, and oversee the end-to-end integration process from due diligence to full operational handoff.

What you'll do:

  • Advise corporate development, operations, and executive leadership on partnership deal structures and operational capabilities.
  • Identify potential friction points in partnerships and develop solutions to mitigate them.
  • Build and nurture relationships with partner organizations to understand their structures, processes, and key details.
  • Serve as a champion for business priorities and ensure alignment between Upperline and its partners.
  • Develop standard operating procedures (SOPs) for new market implementations, ensuring scalability and consistency.
  • Lead the end-to-end integration process for new clinics and partnerships, ensuring timely and efficient transitions.
  • Own multiple cross-functional workflows, including technology, training, and clinical care models.
  • Manage project timelines, deliverables, and milestones to ensure on-time and orderly launches.
  • Participate in tool development to streamline integration management.
  • Track key stages of the integration pipeline, identifying risks and opportunities, and resolving issues promptly.
  • Collaborate with business operations, clinical operations, finance, legal, and corporate development teams to facilitate successful integrations.
  • Ensure effective communication and alignment among diverse stakeholders, including company leaders, third parties, physicians, and office staff.
  • Analyze and refine integration processes to enhance efficiency and effectiveness for future launches.
  • Develop and implement workflow automation solutions to improve scalability and reduce manual effort.

Required Skills and Competencies:

  • Bachelor’s degree or higher in Business Administration, Healthcare Management, or a related field.
  • 3-5 years of experience in integration management, project management, or similar roles.
  • Demonstrated success in managing large-scale integration projects, preferably in a value-based care healthcare organization.
  • Experience with partnership or affiliate models and risk-based arrangements.
  • Advanced proficiency in Microsoft Excel and PowerPoint, including data analysis and presentation skills.
  • Exceptional organizational and project management skills, with the ability to manage multiple projects and competing deadlines.
  • Strong interpersonal and communication skills, with the ability to influence stakeholders and drive alignment.
  • Strategic thinker with a results-driven mindset and the ability to thrive in a fast-paced, high-growth environment.
  • Familiarity with project management software and tools.
  • Willingness to travel 30%-40% to Nashville and various markets in the U.S.
