Job summary

The Springs at Willowcreek in Oregon seeks a compassionate Director of Health Services (DHS) to lead our Health Services team across Assisted Living and Memory Care, guiding clinical practices and ensuring safe, person-centered care. The DHS will oversee resident care, guide physician orders, and coach staff for clinical excellence.

This role requires strong leadership, regulatory knowledge, and a commitment to quality, compassionate care for older adults in a supportive community setting.

Qualifications

• Active Oregon LPN license.

Responsibilities

• Lead Resident Care: Oversee health services for residents in Assisted Living and Memory Care, ensuring care is individualized and responsive.
• Provide Clinical Oversight: Conduct resident assessments and guide the implementation of physician orders and nursing care.
• Develop the Team: Train, coach, and support Health Services employees, building confidence and clinical knowledge.
• Monitor Resident Well-Being: Review clinical documentation, charting, incidents, labs, and pharmacy recommendations.
• Coordinate Care: Partner with residents, families, physicians, pharmacies, and other care team members.
• Support Service Planning: Ensure service plans address health needs and are updated as needs change.
• Ensure Compliance: Guide clinical practices in accordance with Oregon regulations and nursing standards.

The Springs at Willowcreek is seeking a compassionate, experienced Director of Health Services (DHS) to lead our Health Services team. This is an opportunity to make a meaningful impact on the lives of residents while developing and supporting the people who provide their care.

In this role, you’ll oversee resident care across Assisted Living and Memory Care, guide clinical practices, and help ensure our community delivers safe, thoughtful, person-centered care. We’re looking for a nurse who leads with sound judgment, builds strong relationships, and is committed to doing the right thing for residents and team members.

What You’ll Do

• Lead Resident Care: Oversee health services for residents in Assisted Living and Memory Care, ensuring care is individualized, responsive, and consistent with each resident’s needs.


• Provide Clinical Oversight: Conduct resident assessments and guide the implementation of physician orders and nursing care.


• Develop the Team: Train, coach, and support Health Services employees, building confidence, accountability, and clinical knowledge.


• Monitor Resident Well-Being: Review clinical documentation, alert charting, incident reports, laboratory results, and pharmacy recommendations to identify concerns and ensure appropriate follow-up.


• Coordinate Care: Partner with residents, families, physicians, pharmacies, and other care team members to support continuity of care.


• Support Service Planning: Ensure resident service plans address health needs and are updated as needs change.


• Ensure Compliance: Guide clinical practices in accordance with Oregon regulations, applicable nursing standards, and community policies.

What You’ll Bring
