Job summary

Alliance Health is seeking a Senior Director, Community Integration and Transition to lead statewide efforts for Transition to Community Living (TCL) and Community Health Worker models. The role emphasizes strong stakeholder partnerships, data-driven decisions, and coordinated transitions across departments.

The position supports a primarily remote schedule with occasional in-office gatherings. You will guide program development, ensure DOJ settlement alignment, and work with DHHS/state partners

Qualifications

• Master’s degree in Human Services, Social Work, Public Health, or a related field from an accredited college/university.
• Five years of progressive experience with behavioral health population transition services and five years of progressive management experience.
• Clinical licensure is preferred.

Responsibilities

• Oversee Unit Operations and set vision, policies, and data expectations for TCL and Community Health Worker models.
• Coordinate messaging across teams to ensure consistency with goals and stakeholder needs.
• Lead communication workstreams for major transitions and new program launches.
• Prepare executives and external stakeholders with reports, briefings, and presentations.
• Collaborate across departments to ensure compliant and timely implementation.
