Job summary

Horizon Home Care & Hospice, Inc. is seeking a Hospice Home Health Aide to provide personal care and supportive services in patients' homes, ensuring safety, dignity, and comfort for individuals and families during a meaningful time.

You will work with an interdisciplinary team, supported by strong training resources and a culture rooted in kindness, respect, and service, making a real difference in end-of-life care.

Qualifications

• Current, valid RN license in good standing in Wisconsin.
• BSN degree required; MSN preferred.
• 5 years of healthcare leadership experience required.
• Home health and/or hospice experience preferred.
• Experience in quality improvement, infection prevention, and regulatory compliance required.

Responsibilities

• Provide personal care and meaningful support in patients' homes, helping each person feel safe, respected, and cared for.
• Collaborate with an interdisciplinary team to coordinate care, ensuring comfort and dignity for patients and families.
