Responsibilities

• Coordinate client appointments from referral through intake, assessment, and ongoing services.
• Serve as the primary contact for clients and families regarding appointments, referrals, and care coordination.
• Coordinate care across internal programs and external providers, including referrals, intake, insurance verification, and follow-up.
