Job summary

Vibra Hospital of Boise seeks a Director of Case Management to lead the hospital's case management program. The role focuses on ensuring high-quality, cost-appropriate patient services and adherence to Joint Commission standards.

Must have active RN or Social Work license (or equivalent), 3+ years of hospital case management, and BLS certification. Preference for ACM/CCM and prior LTAC management experience. Strong leadership and communication skills are essential.

Qualifications

• Current, valid license to practice as RN or Social Worker; OR license to practice as Respiratory Therapist, Physical Therapist, Occupational Therapist, or Speech Therapist with ACM/CCM certification.
• Minimum three years of hospital case management experience.
• Current BLS certification from a Vibra-approved vendor.

• Monitor quality patient standards, patient and payor satisfaction levels.
• Assess, organize, develop, implement and evaluate the case management program.
• Provide case management services to patients with medically complex conditions and rehab needs.

We are seeking a Director of Case Management to join our team!

Hospital Details

Vibra Hospital of Boise, located in Boise, ID, is a 40 bed Critical Care Hospital, providing acute care services to patients recovering from serious illnesses or injuries. Often our patients require additional critical care services for medically complex conditions such as trauma, infectious diseases, wound healing, cardiovascular disease, stroke, amputation and ventilator weaning. As a smaller, specialized hospital, we offer an opportunity to work in a fast-paced and friendly work environment where you really get to know your patients and their families.

Responsibilities

Responsible for monitoring quality patient standards, patient and payor satisfaction levels, and for ensuring that patients are receiving prioritized, cost-appropriate and quality services. Will assess, organize, develop, implement and evaluate the case management program and activities in accordance with current federal, state and local standards, guidelines and regulations and the Joint Commission standards. Demonstrates knowledge and skills necessary to provide case management services to the patient population experiencing medically complex conditions, general medical-surgical conditions with rehabilitation and wound care needs.

Required Skills

• Current, valid, and active license to practice as a Registered Nurse, or Social Worker in the state of employment, or Master's Degree in Social Work required; OR current, valid, and active license to practice as a Respiratory Therapist, Physical Therapist, Occupational Therapist, and/or Speech Therapist in the state of employment with ACM and/or CCM certification required.
• Minimum three (3) years of hospital case management experience required.
• Current BLS certification from a Vibra-approved vendor required.

Additional Qualifications/Skills

• Registered Nurse preferred. Previous LTAC experience preferred.
• ACM or CCM and CCDS certification preferred. Previous management experience preferred.
• Strong leader and manager, able to motivate staff to deliver high quality, results-oriented case management services.
• Strong written and oral communication skills.
• Assertive and personable with the ability to balance the complex challenge of delivering quality, cost- appropriate, and service-oriented outcomes.
• Demonstrates comprehensive knowledge of the principles and concepts of case management and applicable standards of voluntary and regulatory agencies for review activities.
• Proficient in collecting and retrieving material from medical records.
• Knowledgeable in utilization review, medical terminology, appropriate levels of care, treatment, modalities, statues, and healthcare delivery models.
• Interpersonal relationship abilities must be highly refined with negotiation and problem solving skills at a very high level.
• Must possess strong clinical assessment and process skills. Ability to project a professional image.
• Knowledge of regulatory standards and compliance requirements. Strong organizational, prioritizing and analytical skills.
• Ability to make independent decisions when circumstances warrant.
• Working knowledge of computer and software applications used in job functions.
• Freedom from illegal use of and effects of use of drugs and alcohol in the workplace.
