Responsibilities

• Lead and manage daily hospital Case Management operations.
• Oversee patient care coordination, discharge planning, and continuity of care.
• Ensure optimal utilization of clinical resources and achievement of department outcomes.
• Provide executive leadership, SME guidance, team building and change management.
• Direct staffing, onboarding, competencies, and resource allocation to meet goals.
• Ensure timely, high-quality care coordination, discharge planning, and documentation.
• Analyze data and drive sustainable performance improvements.
• Ensure compliance with policies, SOPs and regulatory requirements.
• Collaborate with CMO, CNO, and senior leaders across divisions.

Job description

• Leads and manages all daily hospital Case Management department operations.
• Oversees patient care coordination, discharge planning, and continuity of care.
• Ensures optimal clinical resource utilization and achievement of department outcomes.
• Provides executive leadership, subject matter expertise, team building, and management change.

Key Responsibilities

• Directs case management operations, staffing, skill mix, FTEs, technology, onboarding, and competencies.
• Allocates resources to meet productivity, quality, and staffing goals.
• Oversees core Case Management functions, practices, and standards.
• Ensures timely, high-quality care coordination, discharge planning, and documentation.
• Analyzes operational data and implements sustainable performance improvement initiatives.
• Ensures compliance with organizational policies, SOPs, and regulatory requirements.
• Collaborates with CMO, CNO, ancillary leaders, Ethics & Compliance, Legal, CFO, and Division CM Leader.

Qualifications

Experience

• 3+ years of overall acute care hospital Case Management experience required.
• 2+ years of Case Management leadership experience required.
• Extensive Case Management leadership experience with strong data analysis and action-planning skills.
• Strong physician and post-acute community relationships; excellent communication and leadership skills.

Education

• Bachelor's degree required.
• Master's degree in nursing, Health Administration, or Business Administration - Preferred

License

• Active RN, LCSW, or LMSW license required.
