Lake Charles Memorial Health System is seeking a Director of Clinical Case Management to lead the department, develop and enforce policies, and align case management for patient care efficiency.

You will oversee budgeting, staffing, training, and compliance with CMS standards, licensure, and mandatory requirements, while guiding a team of case managers and social workers to improve outcomes and resource utilization across the hospital.

Formación

• RN or Social Work degree from an accredited college or university.
• Ten years experience as a RN or SW required, five years as a case manager and 3 years in a mid-management position preferred.
• Current Louisiana Registered Nurse license or Social Work license or Multi-State compact license.
• Computer knowledge for use of Internet, Word, Excel Outlook. Knowledge of Sequel an asset.
• Knowledge of CMS Standards related to Utilization Management and Utilization Review as well as Conditions of Participation.

Description Of Position

The Director of Clinical Case Management develops, implements and enforces policies and procedures which support the Hospital's mission, goals and objectives.

Reports directly to VP of Financial Services

Education And Work Experience

• RN or Social Work degree from an accredited college or university.

Physical Demands/Work Environment

• Work requires a variety of physical activities, including moving about within and outside of all hospital properties for long periods of time.
• Must be able to respond quickly and effectively to emergency and non-emergent situations.
• May be required to assist in controlling disorderly conduct or combative patients.
• Must be able to exchange accurate information with patient, family, peers and medical personnel.
