Job summary

Community Health Systems' Merit Health River Region is seeking a Director of Care Management who will oversee the Care Management and Resource Management programs, ensure regulatory compliance, and drive efficient discharge planning and patient-centered care.

You will lead a multidisciplinary team, collaborate with physicians and community resources, and set policies to improve outcomes while aligning with Joint Commission standards and state laws.

Qualifications

• Bachelor's Degree in relevant field required.
• Seven (7) plus years of direct leadership experience in lieu of a Bachelor's degree.
• Master's Degree preferred.
• 3-5 years of experience in closely related field with Bachelor's degree required.
• 3-5 years of previous leadership experience preferred.

Responsibilities

• Oversee the implementation and maintenance of an effective and compliant Care Management program that meets regulatory and accreditation standards.
• Provide leadership and direction to staff, ensuring accountability for duties and adherence to roles within their scope of practice.
• Conduct chart reviews to evaluate severity of illness and intensity of services for admissions, continued stays, and discharge planning.
• Coordinate discharge planning and social services functions to ensure compliance with federal and state laws and Joint Commission standards.
• Act as liaison to community agencies, providing resources and services for post-hospital placement and support.
• Participate in Patient Care Conferences and Length of Stay meetings to support efficient care delivery.
