Job summary

LEAP, a Maine nonprofit serving individuals with IDD and Autism, seeks a Director of Care Coordination to lead Case Management and stand up Community Resource Coordination as Lifespan Waiver implementation advances. This is a full-time, salaried role based in Farmington with hybrid flexibility and leadership oversight of regulatory quality and program development.

Qualifications

• Bachelor's degree in social work, human services, public administration, healthcare administration, business, or related field.
• At least five years of progressively responsible experience in case management, care coordination, IDD/Autism services, HCBS or related field.
• Leadership, supervision, program development, quality, or compliance experience.
• Ability to interpret regulatory requirements and translate to operations.
• Valid driver's license and ability to meet LEAP driving requirements.
• Background checks and LEAP hiring requirements.

Responsibilities

• Lead CRC development and Lifespan readiness.
• Provide senior leadership for Case Management.
• Drive quality, compliance and audits.
• Develop people, programs and workforce planning.
• Build partnerships with state agencies, providers and communities.
• Contribute to LEAP's Senior Leadership Team.
