Job summary

Diverge Health is seeking a Director of Provider Network Development to expand our primary care network across Illinois, Chicago and Southern Illinois. You will build relationships with independent practices, negotiate contracts, and partner with health plan partners to align with Medicaid-focused growth.

This role blends hands-on dealmaking with strategic market development, reporting to the Senior Director, and offers the opportunity to influence patient access and outcomes through a growing

Qualifications

• Minimum 5+ years in healthcare network development or related growth roles.
• Willingness to commute to physician offices with own vehicle.
• Strong outside sales and networking skills.
• Familiarity with healthcare economics and contract terms.
• Comfort with data and pipeline reporting in a metric-driven environment.

Responsibilities

• Drive growth of Illinois market physician partners and meet individual growth targets.
• Identify and cultivate relationships with independent PCPs to expand network.
• Initiate, negotiate, and finalize provider contracts with PCPs.
• Coordinate onboarding with operations and population health teams.
• Oversee data exchange with partner practices for informed decisions.
• Support transition of relationships to field teams.
• Develop strategies for network development initiatives and contract negotiations.
• Implement long-term growth plans aligned with practice size and geography.
• Deliver excellent customer experience to new leads and existing providers.
• Develop partnerships with FQHCs and large PCP employers.
