Compensation: $96,266 – $179,046 | Comprehensive Benefits Beginning Day One

You will own the full clinical, operational, and financial performance of the agency: day-to-day operations, regulatory compliance, payer credentialing, QAPI, intake and scheduling oversight, and the relationships with hospital and care management partners that drive your referral pipeline. You will function as the senior-most leader on site, supported by clinical supervisors and after-hours RN coverage — and as a valued member of a system-wide post-acute leadership team, not as an isolated administrator buried under paperwork in a one-agency shop.

What You'll Do

  • Direct the agency's clinical, operational, and financial performance — meeting and exceeding national benchmarks on the KPIs that drive HHVBP scoring, OASIS reportable outcomes, and Medicare margin.
  • Lead the agency's day-to-day operations, ensuring clinical supervisor coverage during all operating hours and RN availability 24/7 for patient concerns.
  • Own financial performance: monitor variances, control costs, optimize reimbursement under PDGM, and manage payer credentialing and contract services.
  • Maintain full compliance with state regulators, CMS, CLIA, and accrediting bodies — and lead the QAPI committee that meets quarterly to act on patient safety, infection control, and outcomes data.
  • Partner with hospital leaders, case management, and physician groups to strengthen referral relationships and orchestrate seamless transitions of care.
  • Build, develop, and retain the team — partnering with talent acquisition and HR on hiring, qualification verification, performance, and culture.
  • Step in when needed — clinical supervision, intake, scheduling, quality review, utilization management, or medical records — modeling the leadership style your team will pattern after.

Who You Are

REQUIRED

  • Bachelor's degree in Nursing or a healthcare-related field (an equivalent combination of education and 4+ additional years of relevant experience may be considered).
  • 5+ years of progressive leadership experience running home-based care operations or comparable healthcare operations.
  • Active Registered Nurse (RN) license OR Physical Therapist (PT) license.
  • Demonstrated clinical expertise, leadership ability, and command of community healthcare resources.

PREFERRED

  • Basic Life Support (BLS) certification.
  • Experience operating under PDGM, HHVBP, and OASIS reporting frameworks.
  • Track record of multi-disciplinary collaboration with acute-care and post-acute partners.
  • Familiarity with major home health EHR/EMR platforms and value-based care analytics.
