Qualifications

  • Six Sigma Green Belt LSSGB or Six Sigma Black Belt LSSBB certification.
  • Bachelor's Degree in Nursing, Business Administration, Health Administration or related healthcare field.
  • Master's Degree in Nursing, Business Administration, Health Administration or related healthcare field preferred.
  • 7+ years of experience in a healthcare environment including project management, care coordination, strategy, communications and external vendor collaboration.
  • 5+ years of management or supervisory experience in a complex healthcare setting.

Responsibilities

  • Lead Outsourced Medical Services, the post-acute network, and centralized referral operations.
  • Design and implement integrated strategies, governance frameworks, and operating models.
  • Translate operational data into actionable insights and drive performance accountability.
  • Advance patient-centered outcomes for funded and Financial Assistance Program populations.

Job Summary

The Director, External Care Transitions Network provides enterprise leadership for Outsourced Medical Services, the post acute network, and centralized referral operations, ensuring seamless, equitable transitions of care beyond the health system. This role designs and executes integrated strategies, governance frameworks, and operating models that optimize access, quality, capacity, and cost effectiveness across a diverse external provider landscape. By partnering with clinical leaders and cross functional stakeholders, the Director translates complex operational data into actionable insights, drives performance accountability, and advances patient centered outcomes for both funded and Financial Assistance Program populations.


Minimum Qualifications

License & Certifications:


Six Sigma Green Belt LSSGB (Or) Six Sigma Black Belt LSSBB


Degrees


Bachelor's Degree Nursing, Business Administration, Health Administration or related healthcare field


Master's Degree Nursing, Business Administration, Health Administration or related healthcare field Preferred


Work Experience

7 Years of Experience in a healthcare environment, including project management, care coordination, strategy, communications, and working with external vendors.


Management Experience


5 Years of Experience in a Management or Supervisory role within a complex health care environment
