Job summary

Catholic Charities Brooklyn and Queens, Inc. seeks a Director of Homebound Services for Older Adults to oversee case management and home-delivered meals for homebound seniors in the community. You will supervise staff, ensure quality services, and meet contractual obligations with funders.

You will develop program strategies, monitor outreach, and ensure timely reporting while advocating for elder needs across NYC aging initiatives.

Qualifications

• Master’s in social work or Mental Health Counseling (or psychology) required.
• Licensed in the field (LCSW, LMSW, LMHC) preferred.
• Minimum 5 years’ experience working with the elderly.
• At least 5 years supervisory or administrative experience.
• Excellent communication and interpersonal skills; proficient in Microsoft Office and Outlook.

Responsibilities

• Manage the overall operations of case management and meals programs day by day.
• Ensure proper and timely reporting per funding and agency requirements.
• Plan and strategize core program function and coordinate continuous quality improvement.
• Establish partnerships with agencies and community resources.
• Ensure client satisfaction measurements are in place.
• Provide leadership to staff and supervise through regular meetings and case reviews.
• Adhere to agency and funding source requirements and policies.
• Provide direct services to clients in case reviews and crisis intervention as needed.
• Coordinate referrals and follow ups to internal and external services for older adults.
• Participate in grievances resolution and quality assurance processes.
• Maintain relationships with service providers and participate in meetings as needed.
• Oversee funding source software systems for programs according to NYC Aging guidance.
• Develop and maintain a quality assurance plan per policies.
• Hire, train, and evaluate staff per agency guidelines.
