Job Summary

The Director of Community Health Programs leads and supports the strategy, design and implementation of Mass General Brigham system strategies for community-based programs to reduce health inequities across Mass General Brigham communities. This role provides leadership for program priorities and execution toward improving health outcomes for patients and community residents. Achieving success in this role will require:

  1. A commitment to developing programs for improving outcomes for historically marginalized populations and a deep understanding of the social determinants of health and their impact on health outcomes.
  2. In depth knowledge of the priorities defined by the system community health needs assessments and community health improvement plans to inform community-centric program development.
  3. Taking a data driven, research-backed, community-informed approach that builds on national best practices and benchmarks, leverages key partnerships and collaboration, and is grounded in measurement & evaluation.
  4. Expertise in program design, implementation, and evaluation.

The role requires close collaboration with MGB community health and health equity leaders and includes oversight of program teams to carry out the work. This position reports to the Mass General Brigham VP of Community Health. The Director will provide leadership of community health programs to ensure measurable outcomes. These include community programs dedicated to maternal health equity, youth programs, community engagement, and programs addressing key social determinants of health. The overarching goal of all initiatives will be to demonstrate a reduction in health inequities and a measurable improvement in community health outcomes.

General Responsibilities

  1. Leads Community Health programs to improve health outcomes and address social determinants of health.
  2. Develops and executes system-prioritized chronic diseases prevention and management and CHNA-prioritized social risk factors to bring high-quality, equitable community programs to historically marginalized populations in Mass General Brigham primary services areas.
  3. Understands the priorities defined by the system community health needs assessments and community health improvement plans.
  4. Leverages knowledge of project dependencies and timelines to execute system strategies.
  5. Provides leadership to community health improvement staff executing community-centric programs.
  6. Other duties as required.

Program Responsibilities:

  1. Provides leadership over development, implementation, and evaluation of system-wide community-based programs.
  2. Conducts comprehensive assessment of existing and planned programs for feasibility, successful execution (including demonstrating measurable outcomes), and sustainability.
  3. Engages in partnerships across sectors and across disciplines to design programs to improve community health.

Qualifications

  • Bachelor’s degree required in public health, health sciences or related field, Masters preferred.
  • Minimum of 5 years of experience in leading complex projects and teams in community-based settings.
  • 3-5 years prior experience managing cross-functional teams and projects.

Skills and Abilities:

  • Strong knowledge of program development and implementation.
  • Understanding of the triennial community health needs assessment and community health improvement planning processes.
  • Understanding of the Mass General Brigham System policies and procedures, community/ political landscape, and market/ industry position.
  • Strong leadership and mentoring skills.
  • Demonstrated ability to formulate and execute strategy.
  • Experienced in planning, organizing, prioritizing, delegating and supervising.
  • Proficient in exercising initiative, judgment, problem solving and decision making.
  • Exceptional communication skills, including strong verbal, written and interpersonal skills.
  • Ability to build and manage relationships in a highly complex and changing environment.
  • Skilled at facilitation and building consensus and navigating sensitive issues.
  • Skill in negotiation with stakeholders, vendors and other parties.
