Job summary

MyPath is seeking dedicated care professionals to support children, adolescents and adults with special needs across community and home-based settings. You will perform direct care services in alignment with the Direct Support Professional (DSP) essential functions, document care activities, and monitor client well-being.

Collaborating with supervisors and the medical team, you will help implement individualized plans and engage clients in activities.

Responsibilities

• Performs direct care services in accordance with the Direct Support Professional (DSP) essential functions as documented in the Position Description.
• Produces regular written updates and documentation of care activities.
• Monitors health status of clients and reports changes to the client’s medical team.
• Assesses client needs and supports implementation of individualized service plans.
• Fosters engagement through activities and community participation as appropriate.
