Job summary

Mypathcompanies seeks a dedicated individual to monitor and improve service and care provisions across assigned locations. You will ensure plans are followed, document activities, report concerns, and support supervisors in implementing changes for positive outcomes.

The role requires overseeing health status, physician orders, and dietary planning, while managing budgets, scheduling, and training staff to meet company standards.

Qualifications

• Experience delivering direct care services in a professional setting.
• Knowledge of confidentiality, licensing standards and HR policies.
• Ability to provide ongoing feedback and conduct performance evaluations.

Responsibilities

• Monitor service and care provisions for quality and effectiveness.
• Monitor and document care per policy; report concerns to Supervisor.
• Develop and implement changes to service plans under supervision.
• Track client health status and coordinate with medical teams as needed.
• Review physician orders and treatments for accuracy and compliance.
• Develop weekly meal menus considering dietary needs and preferences.
• Manage budget, shopping for groceries and supplies, and petty cash.
• Create monthly activity calendars and arrange community and home-based activities.
• Assess staff performance and provide coaching and corrective actions as needed.
• Maintain staff morale and encourage open communication.
• Generate weekly work schedules to optimize coverage and minimize overtime.
• Ensure proper timekeeping and staffing coverage across shifts.
• Conduct service location and client-specific training for staff.
• Maintain the physical condition of the location and report hazards or repairs.
• Keep Company vehicles clean and coordinate maintenance when needed.
• Ensure confidentiality and professional conduct at all times.
• Report abuse, rights violations, harassment, or fraud to management.
