Job summary

Qualifications

• 1–2 years of related employment experience
• Criminal background check and fingerprinting clearance
• Valid driver's license with fewer than 4 points

Responsibilities

• Engage residents in activities of daily living and support personal needs such as hygiene, meals, and medication.
• Observe, document and report changes in health or behavior per plans.
• Perform household tasks and assist residents in selecting activities.
• Ensure safe transportation and accompany residents to programs or appointments.
• Prepare progress notes on medications and daily activities as required.
• Communicate with residents' families and participate in staff meetings.
