Zusammenfassung

The Society is seeking a Direct Support Professional in Ohio to provide daily care and assistance to individuals served, following Ohio Individual Support Plans. The role includes ADL support, transportation, safety, and documentation.

Responsibilities include medication administration per requirements, collaboration with staff, and promoting independence and community integration. This is a full-time or part-time on-site position with some shifts and holidays required.

Qualifikationen

• Must be 18 years of age or older.
• Must have a high school diploma or GED.
• Must be able to follow verbal and written instructions.
• Must be able to communicate in English including both spoken and written.
• Must obtain and maintain certification in Medication Administration and Health Related Activities, First Aid, CPR, PMT and other courses as required.
• Must have a valid Ohio Driver’s License, good driving record, and carry the proper liability insurance amounts required by The Society.

Aufgaben

• Provide day to day interactions, care and assistance for individuals as outlined in Ohio ISPs.
• Assist with Activities of Daily Living (ADLs) and promote independence in domestic tasks.
• Ensure safety, cleanliness and health of individuals and environment.
• Perform medication administration tasks and related documentation per policy.
• Coordinate transportation and community integration.
• Communicate effectively with individuals, families, and team members.
