Job summary

Mentor Community Services, a part of the Sevita family, is seeking a Direct Support Professional for Supported Living Services. Based in community settings, you’ll work with our clinical team to support therapeutic and behavioral plans, helping individuals live as independently as possible.

Qualifications

• High School Diploma or GED.
• Must be at least 18 years old.
• Six months of experience in human services preferred.
• Valid driver’s license in good standing.
• Access to a registered vehicle with proof of insurance.
• Successful clearance of background checks.
• Reliable, responsible attitude.
• Compassionate approach and a commitment to quality in everything you do.

Responsibilities

• Transportation and support in the community: outings, medical appointments, and shopping.
• Assist with daily living skills including meal preparation and medication administration.
• Provide guidance on developmental goals from community participation, budgeting, exercise, nutrition, and vocational tasks.
• Support living skills such as personal hygiene, grooming, bathing and toileting.
• Help individuals redirect agitated or aggressive behaviors.
