Job summary

You will assist with daily activities, transportation, and participation in recreational events, while maintaining clear records and communicating with the care team.

The ideal candidate will have strong communication and problem-solving skills and be able to work collaboratively with others to ensure consistent, person-centered support.

Qualifications

• Strong communication and problem-solving skills.
• Ability to multitask and handle a high-volume workload.
• Ability to work collaboratively with other team members.
• Valid driver's license and reliable transportation are a plus.

Responsibilities

• Assist clients with daily activities and personal care.
• Provide transportation to appointments and activities as needed.
• Participate in recreational activities with clients and outings.
• Maintain accurate care records and report changes to the team.
• Collaborate with team members to ensure continuity of care.
