Job summary

Prairie Community Services, Inc. is seeking a Direct Support Provider in Morris, MN. This full-time, on-site role involves supervising and supporting individuals in daily living activities, recreation, and community orientation, including medication management, meal prep, and daily chores.

The position starts at a wage of $16.50–$17.50 per hour. Responsibilities include following person-centered plans, documenting progress, and maintaining safety and well-being of those served.

Qualifications

• Minimum age 17 or as required by licensure.
• Must pass DHS background clearance.
• Must have valid driver’s license.
• Must be able to read, write, communicate and understand English.
• Read, comprehend and follow each person served’s program related plans and procedures.
• Deliver support to person served on principles of person-centered services.
• Administer CPR/First Aid when needed.
• Chart daily program information.

As a Direct Support Provider, you will provide supervision, care and safety to individuals being served that include, but are not limited to, areas of daily living, recreation, community orientation, medication management, meal preparation, laundry, and cleaning. Starting wage is $16.50-$17.50 per hour.
