Job summary

Prairie Community Services, Inc. in Morris, MN is seeking a Direct Support Provider to supervise, care for and support individuals in daily living activities.

Starting wage is $11.41 per hour. Responsibilities include medication management, meal preparation, documentation, and following person-centered plans; this is an on-site night-shift position in Ravenwood Home, Morris, MN.

Qualifications

• Minimum of 17 years of age or as dictated by licensure.
• Must pass DHS background clearance.
• Must have valid driver’s license.
• Must be able to read, write, communicate and understand the English language while conducting company business.
• Read, comprehend, and follow each person served’s program related plans and procedures.
• Deliver support to the person served on principles of person-centered services.
• Administer CPR/First Aid when needed.
• Chart daily program information.
