Job summary

Sevita’s Mentor Community Services division seeks a Program Supervisor to lead a team delivering community-based supports for individuals with intellectual and developmental disabilities. You will help implement ISPs, supervise caregivers, support daily living activities, and coordinate community outings and transportation.

Ideal candidates will have a high school diploma, at least one year of related experience, and strong communication and organizational skills.

Qualifications

• High School diploma or equivalent.
• One year related work experience.
• Must be 18 years or older.
• Current driver’s license, car registration, and auto insurance.
• Other licensure or certification as required by regulatory authority.

Responsibilities

• Facilitate implementation of Individual Service Plans (ISP).
• Supervise a team of Caregivers supporting individuals in the program.
• Provide support with daily activities: meal preparation, medication assistance, personal care.
• Balance direct service with supervision and professional/program activities.
• Facilitate community involvement and transportation to work or other activities.
