Job summary

RHA Health Services, LLC is seeking a Direct Support Professional for periodic full-time work in Nashville. You will support individuals with disabilities and mental health challenges in residential and community settings, following each person’s Individual Support Plan.

You will collaborate with an interdisciplinary team, promote client autonomy, and help with daily living activities, transportation, and documentation. Training in CPR/first aid is provided.

Qualifications

• High School Diploma or GED, or AA in a health-related field.
• Experience working with individuals with developmental and intellectual disabilities preferred.
• CPR and first aid certifications, plus related training is a plus.
• Pre-employment screening including background checks and credentials as required.
• Valid driver’s license and vehicle for transportation duties.

• Providing opportunities for choice and encouraging clients to exercise control over themselves and their environment.
• Helping identify personal outcomes matching interests, culture, and goals.
• Work with an interdisciplinary healthcare team to determine appropriate support methods.
• Implement services and supports per Individual Support Plan to achieve goals.
• Assist with personal plans, goals, and communication needs.
• Transporting clients to day programs, work sites, and community activities.
• Dispensing medications and monitoring dosages as trained.

We are hiring for: Direct Support Professional - Periodic FT. Type: Regular. Provides support for individuals and/or groups of persons with disabilities in residential and community settings. Implements services and supports to address the person’s goals identified through each person’s Individual Support Plan.

Job Responsibilities

• Providing opportunities for choice and encouraging clients to exercise control over themselves and their environment
• Helping to identify personal outcomes that will best suit their individual interests, cultural backgrounds and dreams for the future
• Work with an interdisciplinary healthcare team to identify the appropriate support methods necessary to achieve the stated goals
• Implement services and supports that will assist your client to achieve personal goals and desired outcomes as identified through an Individual Support Plan
• Assist them with the implementation of personal plans, goals, and objectives; working with them to facilitate their communication in terms of needs and requests
• Assisting and instructing on food preparation and house cleaning
• Helping clients to identify and acquire valued social roles, as well as healthy personal and social habits
• Continuously assessing the functional levels for your client, motivating clients and providing consistent reinforcement, assessments, and feedback for all learning activities
• Dispensing medication, monitoring dosages and making notes in charts in accordance with your level of training
• Transporting and accompanying clients to and from day programs, work sites, and community activities
• Supporting clients at vocational sites to help meet their responsibilities
• All other duties as assigned

Job Requirements

• High School Diploma or G.E.D., Associate Degree in a health related area; or two years’ experience in a health related field
• Experience working with individuals with developmental and intellectual disabilities preferred
• Valid drivers’ license and automotive insurance
• Flexible schedule and availability to provide on-call coverage as needed
• Provide CPR after certification. CPR, first aid, NCI and/or MedTech certification, a plus
• Pre‑employment screening: Complete criminal background name checks in the registries (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities), drug testing, education verification and other credentialing based on position requirements, proof of employment history or references (if required)
• Positions that require driving: Proof of driver’s license, driver’s insurance, and vehicle, if required for providing transportation for individuals
