Job summary

ACHIEVE is seeking a Direct Support Professional for overnight shifts in Chittenango, NY. This full-time role provides 24-hour supervision, care, and support to residents in our residence program, including medication administration and daily living assistance.

You will promote safety, independence, and participation in meaningful activities under individual plans, working within OPWDD guidelines and a supportive team environment.

Qualifications

• High School Diploma or GED required.
• Experience in human services or with individuals with functional differences preferred.
• Valid NY State driver’s license.
• Ability to lift and assist with up to 50 pounds.

Responsibilities

• Stay awake during shift and monitor residents and building.
• Provide individualized care per ISP/Life Plan.
• Support safety and well-being of residents.
• Document behaviors, nutrition, and clinical issues for treatment goals.
• Administer medications as prescribed by physician (OPWDD regulations).
• Lead therapeutic activities and document accordingly.
• Assist with activities of daily living (bathing, dressing).
• Document delivery of residential habilitation services per plan and procedures.
• Encourage positive interactions and social engagement.
• Prepare and serve meals,-model healthy eating.
• Assist residents with mobility and daily tasks as needed.

About ACHIEVE's Residence Program

In the residential program, the most caring people are the trained staff who provide 24-hour supervision, care, and guidance to residents. They assist with personal care needs, cleaning, cooking, medications, and activities. The Direct Support Professional Residential Overnight assists in providing self-care training, therapeutic treatments, and assistance in daily living to developmentally disabled individuals attending the agency's respite facility.

Responsibilities

• Must stay awake and not place themselves in a compromising position/place that is conducive to sleep; Continuously assess the status of the physical building and the individuals throughout the duration of the shift.
• Provides individualized care as outlined in the individuals Life Plan, or ISP
• Supports the residents to ensure their safety and well-being
• Observes, documents and communicates pertinent information such as behaviors, nursing, nutrition, clinical issues, etc., to facilitate assessment and development of treatment goals
• Gives medications as prescribed by Physician, per OPWDD regulations
• Administers therapeutic activities, such as physical exercises, occupational arts and crafts, and recreational games to residents and documents accordingly
• Demonstrates and assists with activities such as bathing and dressing to train residents in daily self-care practices
• Documents delivery of residential habilitation services per the individual plan and agency procedures
• Converses with residents to reinforce positive behaviors and to promote social interaction
• Prepares and serves meals and dines with residents to act as a role model
• Assists residents in mobility, hygiene, and any other tasks in which assistance or direction is necessary

Requirements

• High School Diploma or GED required
• Experience working in human services or with individuals with functional differences preferred
• Valid NY State driver’s license with a driving record acceptable to the agency’s insurance carrier
• Ability to lift and assist with up to 50 pounds, including transfers and physical support
