Job summary

All Ways Caring HomeCare is hiring Direct Support Professionals to provide compassionate direct care in clients’ homes. You will assist with personal care, mobility, health monitoring, and daily activities, while fostering independence and dignity.

You’ll implement care plans, support behavior plans, and document progress. The role involves driving clients to appointments and participating in a team-driven, supportive home-care environment.

Qualifications

• Must be 18+ years old
• Background check required
• Driver's license
• High school diploma/GED
• Valid auto insurance

Responsibilities

• Perform personal care tasks including hygiene, grooming, feeding and ambulation.
• Monitor health and administer basic health-related tasks.
• Foster positive relationships between individuals served and their circle of support.
• Encourage self-help activities.
• Implement Individual Service Plan and Behavior Support Plans.
• Manage behaviors by prevention or de-escalation of aggression.
• Keep accurate daily documentation of activities and related plans.
• Transport individuals to activities and appointments.
• Perform other duties as assigned.

Responsibilities

• Performing personal care tasks, including assistance with basic personal hygiene and grooming, feeding, and ambulation, medical monitoring, and health care related tasks


• Fostering positive relationships between individuals served and their circle of support.


• Encouraging self-help activities


• Implements Individual Service Plan and Behavior Support Plans


• Manage Individual's behaviors by prevention or de-escalation of aggression.


• Keeps accurate daily documentation of activities, fire/ disaster drills, Behavior Support Plans, medications, goal and data tracking, as well as other documentation.


• Transport individuals to and from community activities and medical appointments.


• Transporting clients or running errands for clients


• Other duties as assigned.
