Qualifications

Responsibilities

• Assist clients with activities of daily living (ADLs).
• Provide community integration and support.
• Follow Individual Service Plans (ISP).
• Document services and progress notes accurately.
• Ensure client safety and well-being.
• Report incidents and changes in condition.
• Maintain professionalism and confidentiality.
