The Chimes, Inc. is seeking a dedicated Direct Care Worker to support residents with developmental disabilities in home and community settings. You will supervise, train, and assist individuals to reach their personal goals while ensuring safety and dignity in daily activities.

You will follow individual program plans, administer medications, and collaborate with program managers to maintain a supportive living environment. Evening, weekend, and holiday shifts may be required.

Qualifications

• Requires high school diploma or equivalent.
• Must have a valid driver’s license and acceptable driving record.
• Ability to lift, push, and pull to assist individuals (over 50 lbs).
• Ability to work as part of a team.

Responsabilités

• Assist residents in home and community activities to support personal goals.
• Provide supervision and training to adults with developmental disabilities.
• Ensure personalized house environment and individual preferences are respected.
• Teach residents to perform tasks independently per program plans.
• Communicate with day program staff about individual programs.
• Report safety and house operation concerns to supervisor.
• Use approved behavior plans and intervention strategies.
• Be a positive role model for residents and staff.
• Help residents develop independent living skills (mobility, budgeting, hygiene).
• Maintain resident safety and whereabouts during shifts.
• Attend scheduled meetings with program managers.
• Know routine medical procedures and monitor health indicators.
• Administer medications per PMOF/MAR and perform three-way checks.
• Complete records and reports (monthly, incident, seizure, logs, drills).
• Clean and prepare meals per approved menus and diets.
