Job summary

TRUCAREING INC in Fort Wayne, IN is seeking a Direct Support Professional to assist individuals with disabilities in community-based services. Shifts include part-time and full-time, in-person work.

You will support individuals, document progress, transport them, and ensure safety, following program guidelines and reporting incidents as needed. Requires a high school diploma or equivalent, a valid Indiana driver’s license, and the ability to lift 50–75 lbs; CPR/First Aid training is expected

Qualifications

• GED/HS Diploma Equivalent required.
• Weekend availability is required.
• Valid Indiana driver’s license with a safe vehicle and insurance.
• Recent DMV print-out with good driving record.
• Ability to lift 50–75 lbs and pass a post-offer physical.
• Must pass criminal background check and CPR/First Aid training within two weeks.
• Read, speak and write English; able to communicate effectively in writing.
• Desirable: experience with persons with disabilities; higher education; flexible schedule; bilingual.

Responsibilities

• Assist Case Manager in developing goals for individuals we support.
• Provide individualized instruction and support for people with disabilities.
• Implement community-based employment, living, and inclusion services.
• Follow schedules, report changes, and document progress daily.
• Transport individuals and accompany them on public transportation.
• Report incidents promptly and maintain safety and dignity.
• Handle emergencies per program guidelines and assist with daily living activities.
