Qualifications

Responsibilities

• Provide one-to-one support to individuals in homes and communities.
• Assist with daily living skills and independence goals.
• Document progress and maintain HIPAA-compliant records.
• Collaborate with care teams and families for goal achievement.
• Ensure safety and follow individualized support plans.
