Qualifications

• High school diploma or GED preferred.
• 6 months direct support experience required.
• 1-year direct support experience in a DD residential setting preferred.

Responsibilities

• Provides support to residents as outlined in ISPs and Health plans.
• Administers medications according to procedures.
• Completes accurate daily documentation (ISP Notes, T-Logs).
• Supports residents with Activities of Daily Living and daily living tasks.
• Encourages residents to engage with others and participate in community activities.
• Follows Heart Havens policies and procedures.
