Qualifications

Responsibilities

• Provide care, training, supervision and assessment per PCSP
• Report progress daily and as required by PCSP
• Assist with meals planning and preparation
• Support clients in social and communication skills and community integration
• Maintain a healthy living environment and safety compliance
• Transport clients or arrange transportation
• Communicate with inter-departmentally to ensure service delivery
• Participate in the client’s Person-Centered Support Plan team
