Qualifications

• Experience delivering person-centered supports to individuals with developmental disabilities.
• Ability to document accurately using electronic health records.
• Strong communication and teamwork skills.
• Knowledge of HIPAA and confidentiality requirements.

Responsibilities

• Provide direct care and supervision in residential, day program, or in-home settings.
• Implement Life Plan goals and document progress.
• Observe, assess, and report changes in individuals' status.
