Job summary

Qualifications

• High school diploma or GED is required.
• At least 6 months of direct-care, healthcare, residential, or related experience.
• Must be 18 years or older.
• Ability to communicate clearly and document accurately in writing.

Responsibilities

• Provide person-centered assistance based on each individual's service plan and health/safety protocols.
• Assist with daily living activities and promote independence.
• Maintain a safe, clean, and home-like environment.
• Prepare meals and manage household tasks.
• Document services and observations accurately in Therap.
• Support community participation and appointments.
