Job summary

Rising Ground, Inc. is hiring an IDD-Direct Support Professional - Driver in Queens, NY. This full-time role supports individuals with disabilities across Westchester & NYC, driving as needed and providing person-centered care.

The DSP assists with daily living activities, community integration, and health-related tasks under RN oversight. Immediate start date with extensive scheduling options and benefits.

Qualifications

• High School Diploma or GED is required.
• Valid NYS driver's license preferred.
• Ability to work independently and be self-directed.
• Proficient in using technology for billing data.
• Strong written and verbal communication skills.
• Flexible and able to meet scheduling needs.
• Experience with OPWDD or supporting people with disabilities is a plus.

Responsibilities

• Support individuals to achieve personal goals per Life Plan and preferences.
• Provide supervision and individualized support based on each person's needs.
• Escort individuals to community activities, appointments, and services.
• Safely operate agency vehicles for transportation and program needs.
• Assist with meal planning, grocery shopping, and daily living skills.
• Administer medications when authorized, per AMAP and RN oversight.
• Maintain a clean, safe, home-like environment.
• Comply with OPWDD, NYS Justice Center, and Rising Ground policies.
• Document progress notes, billing records, and clinical data timely.
