Job summary

S and O, Inc. is seeking Direct Support Professionals to provide care to individuals in Arizona DDD group homes.

You will support daily living activities, follow Individual Service Plans, and work with medical care teams to ensure safety and well-being. Requirements include a High School Diploma or GED, 18+ years, valid driver's license, background clearance, and ability to lift up to 100 pounds; training and fingerprint clearance in Arizona are part of the process.

Qualifications

• Six months of experience in human services preferred.
• Valid Driver's license in good standing.
• Successful clearance of background checks.

Responsibilities

Job DetailsJob Location: Phoenix, AZ 85043S and O, Inc. provides care services to individuals in the Division of Developmental Disabilities (DDD) group homes in Arizona. Our homes are ADHS- licensed, DDD Group Homes are open 24 hours a day 7 days a week and 365 days a year. We are currently seeking great people who wants a rewarding career to join our dynamic team. We are currently hiring Direct Support Professionals.

Our direct care professionals will work with medical care teams and case workers to provide care to our members ranging from basic household tasks to accompanying the members to their doctor’s appointment. Additional tasks will include:

• Providing assistance, monitoring and/or training to individuals in the area of self-care, self-direction, independence, mobility and learning as directed by the Individual Service Plan (ISP) in order to enhance each individual's skill development, health, freedom and personal dignity.
• Maintain cleanliness of environment
• Possesses effective communication skills
• Assists in behavior management/Crisis intervention. Provides health care including, but not limited to, medication administration, nutrition, allergies, meal planning, etc. Assists with safety skills including but not limited to basic first aid, fire drills, handle emergency situations
• Maintains confidentiality and privacy
• Meets requirements of federal, state, and local organizations. Processes and maintains meticulous record keeping
• Maintains optimum attendance and reliability
• Follows time and attendance guidelines and policies associated with time tracking
• Practices good judgment and has ability to evaluate and anticipate consequences
• Additional duties assigned as necessary

• Must be able to frequently lift/move up to 10 pounds and occasionally lift and move more than 100 pounds
• Ability to complete training programs and successfully pass associated tests
• Ability to obtain a Level 1 Arizona Fingerprint Clearance Card and pass a background check as required by state and federal regulations
