Creative Foundations is hiring Direct Support Professionals to support individuals with developmental disabilities in home and community settings. You will provide one-on-one care, assist with daily living skills, and promote independence while documenting progress and communicating with the care team.

Requirements include a high school diploma, background check, required training, a valid driver’s license, and a compassionate, reliable approach to support in clients’ homes and communities.

Qualifications

• High school diploma or equivalent.
• Successful background check and required training.
• At least 21 years old.
• Med Cert 1.
• Valid driver’s license and reliable transportation with proof of insurance.
• Ability to work safely and respectfully in clients’ homes and communities.
• Compassion, reliability, and a desire to make a positive impact.

Responsabilités

• Provide one‑on‑one support to individuals with developmental disabilities in home and community settings.
• Assist with daily living skills (cooking, cleaning, personal care, transportation).
• Encourage participation in social and community activities and promote independence.
• Document care, activities, and progress accurately.
• Communicate with team members to ensure consistent, quality support.
