Job summary

REM Community Services is seeking a direct support professional to assist individuals in residential settings with daily living, health maintenance, and community integration. You will document progress, support social and skill development, and help with transportation and activities.

Responsibilities include recording progress, managing simple purchases within budget, and maintaining confidentiality while working closely with families and case managers.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.

Responsibilities

• Provide direct support per individual service plans, including socialization and health maintenance.
• Assist with daily activities: personal care, housekeeping, transportation, and community integration.
• Accurately document progress and review records to stay aligned with service plans.
• Manage expenses and purchases within the individual's budget and preferences.
• Maintain confidentiality and professional relationships with individuals and families.
• Report any safety concerns or incidents per policy.
