Responsibilities

• Provide care and report progress daily per PCSP
• Attend to personal needs: feeding, bathroom, dressing, medications, first aid
• Assist with meal planning, shopping and preparation
• Maintain a healthy living environment and address safety concerns
• Assist with and/or monitor housekeeping tasks in client residences
• Transport clients or arrange transit
• Help clients develop social and communication skills and community integration
• Participate as a member of the Person-Centered Support Plan team
