Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.

• Working knowledge of computers.

Responsibilities

• Deliver support per individual service/program plans, including socialization and health maintenance.
• Assist with daily activities: Personal care, housekeeping, transportation, and community integration.
• Accurately documents progress and activity; maintains confidentiality.
• Maintains healthy and professional relationships with individuals, families, guardians and case managers; follows Sevita Customer Service Standards.
