Job summary

Southwest Behavioral Care, Inc. in Monessen, PA is seeking a part-time caregiver to aid program clients daily by implementing Individual Service Plans and supporting resident activities.

The role requires a High School Diploma or GED, a valid Pennsylvania Driver’s License with a safe driving record, the ability to use an electronic medical records system, and flexibility to work daylight, afternoon, or midnight shifts including weekends and holidays.

Qualifications

• High School Diploma or GED required.
• Valid Pennsylvania Driver’s License with safe driving record.
• Ability to utilize Electronic Medical Records (EMR).
• Ability to transport oneself and residents in an insured vehicle.
• Criminal History Clearance and FBI fingerprint clearances required.

Responsibilities

• Provide hands-on instruction to clients to implement Service Plans.
• Conduct recreational and social activities with residents.
• Participate in staff training to develop skills for assisting clients.
• Maintain safety, cleanliness and proper condition of the residential site, respond to emergencies.
• Adhere to all policies, laws, regulations and confidentiality requirements.

OVERALL FUNCTION:

Aids program clients daily with the implementing of Individual Service Plans.

ESSENTIAL FUNCTIONS:

• Provides hands-on instruction to client to implement Service Plans and client development.
• Conducts recreational and social activities with residents, as assigned.
• Participates in regular staff training to develop basic knowledge and skills necessary to assist clients.
• Maintains proper condition, cleanliness and safety of residential site, and responds appropriately to emergency situations.
• Other duties as required.
• Adheres to all policies, laws, regulations and codes of ethics and confidentiality as outlined by federal and state laws, as well as agency policies and procedures.

Qualifications

• High School Diploma, GED or any equivalent combination of skills, experience and education may be considered at the discretion of the Agency.
• Must have a valid Pennsylvania Driver’s License and safe driving record
• Ability to utilize electronic medical records.
• Ability to transport oneself and/or residents in an insured vehicle in the performance of essential functions.
• Must successfully obtain Criminal History Clearance from PA State Police, Child Abuse Clearance from Department of Human Services, and FBI Fingerprint Clearances.
• Must not be excluded from participation in Medicare, Medicaid, or other federal health care program.
• Looking for individuals who have the ability to work daylight, afternoon and midnight shifts, as well as working on weekends and holidays
